Provider Checklist for New Clients

Checkbox   Greet the client in a friendly manner.
Checkbox   Ask the client what method they have in mind.
Checkbox   Ask the client about previous method use and satisfaction with and feelings about the method (for example, problems using the method, side effects).
Checkbox   Ask the client about their reproductive intentions and their ideal family size.
Checkbox   Ask the client about their breastfeeding status.
Checkbox   Rule out pregnancy.
Tip: Family Health International's Pregnancy Checklist is a helpful tool.
Checkbox   Ask the client about STI symptoms and assess client's likelihood of exposure to STIs including HIV/AIDS.
Checkbox   Discuss a range of methods with the client based upon their current needs.
Checkbox   Explain how to use the new method correctly and consistently.
Checkbox   Counsel the client about possible side effects.
Checkbox   Ask the client what they are doing to prevent STIs and HIV/AIDS and discuss dual protection choices (how condoms can be used in addition to any another family planning method to prevent transmission).
Checkbox   Ask the client if they have any questions or concerns.
Checkbox   Inform the client that they can switch family planning methods in the future if needed.
Checkbox   Give the client plenty of supplies. For example, if the client uses OCs, provide a full year's supply (13 packs), if possible.
Checkbox   Ask the client for their contact information in order to complete a clinic record or in case the clinic needs to follow-up.
Checkbox   Agree on a date for a return visit and assure the client they are welcome to come back anytime.
